Office Information

Office & Appointment Details:

First, decide whether you want a Phone or Office Appointment (because the preparation is different).

FIRST VISIT: At least 2 hours (depending on complexity) (Fee $300)

Click the Book An Appointment Online button.
Create an account if you do not yet have one, or log in,to see available appointments.
Then choose appointment type, date & time.

Before your First Appointment, please download and complete all 3 New Patient Paperwork PDF Forms (to save us time during your visit), and bring them to your first visit (or email them to me before the visit). NOTE: Please fill out the 3-Day Diet Diary over 3 days in advance, rather than from memory at the last minute.

  • Please do not send cell-phone photos of documents -- they are huge files with low resolution. (If you need a reliable address to send paper documents to, please email me far enough ahead of time.)

  • Please download my Diet for Good Health handout, for your own reference.

  • Bring any relevant outside medical records. (Just the important stuff, not everything)

  • Bring your current medications and nutritional supplements for review.

  • Bring a list of your questions and symptoms.

  • Take notes during the visit. (We discuss a lot of information.)

Toward the end of each visit, I will discuss recommended lab tests, nutritional supplements, dietary and lifestyle changes, estimated timelines and costs involved, and answer your questions.

For Phone Appointments, I will call you at that specified time (Pacific Time Zone):

  • Send me your paperwork before the visit in PDF form (see above). (To send secure documents to me, go to /Contact/).

  • Have your credit card information handy, and be prepared to take notes. You should have any relevant medical records with you for reference.

RETURN VISITS: Usually 1 hour (depending on complexity) (Fee $100)

Click the Book An Appointment Online button. (Log in, but don't create a duplicate account if you have previously registered there; you can reset a forgotten password.) Then choose appointment type, date & time.

  • Bring a list of your questions and symptoms to discuss, & any needed paperwork.

  • Bring any new questions, and take notes during the visit.

  • We will review progress, lab reports & treatments, and recommend program changes.

For Phone Appointments, I will call you at that time (Pacific Time Zone):

Office Policies:

  • Payment is due at the time of service (by Credit Card or Check).

  • Cancellations: If you are unable to keep your scheduled appointment, please notify me at least 24 hours before your scheduled time.

  • I cannot manage medical emergencies. If you have a medical emergency, you must contact your primary care physician or dial 911!
